Formal Admission Acceptance Form
It is required that each candidate formally accept the invitation from Lamar University College of Education and
Human Development as their Educator Preparation Program. After reading the Offer of Formal Invitation, please
initial and sign this signature page. The signature page and the Offer of Formal Admission must then be
submitted to the Admissions Pathway in Portfolium as the final step of acceptance. This submission must be
done prior to enrollment in the first course – EDLD 5311. Please initial the statements below indicating your
understanding:

I understand to seek Texas Certification I must complete all required Internship/Practicum
activities/hours and field experience observations on a K-12 Texas Education Agency approved campus.
I understand to seek Texas Certification I must have a valid Texas Educator Certificate and two
complete years of service with a K-12 state accredited school listed on my Service Record.
I understand I must successfully pass the required Principal 268 and PASL state exams to be
recommended for certification in Texas.
I understand that certification requirements vary for each state, and that changing to an
Internship/Practicum site in another state mid-program may affect my certification eligibility.
I understand that failure to comply with program requirements will prevent me from being approved for
certification by my Educator Preparation Program, Lamar University, in the future upon completion of
my program.
**Although out-of-state students may not apply for Texas Certification, your initials above indicate you are aware
of these state and program requirements prior to enrolling in this Texas Educator Preparation Program.

website. I understand that Lamar University is not responsible for changes in certification requirements for my
state and that I am responsible for confirming certification requirements with the licensing agency in my state.
My signature below verifies that I formally accept the invitation from Lamar University College of Education and
Human Development as my Educator Preparation Program for my program of study. I agree to abide by all
practices, policies, and expectations of the program, and I understand the consequences for failure to do so.
Consequences may include loss of course credit, denial of recommendation for certification, and/or suspension or
expulsion from the program.
