ELIGIBLE NON-CITIZEN STATUS
The US Department of Education could not confirm your status as a citizen or eligible
non-citizen of the United States. Before the Office of Financial Aid can establish your
eligibility for assistance, we must verify your status. We cannot continue processing
your application until we receive the information requested below. Please return this
form and appropriate documentation to:

IF YOU ARE A U.S. CITIZEN, please attach a photocopy of one of the following:
1. Your U.S. birth certificate
2. Consular Report of Birth Abroad (FS-240); Certificate of Birth issued by a foreign service post (FS-545); or
the Certificate of Report of Birth (DS-1350). If you have not taken any of the prior listed documents to
the Social Security Administration (SSA), you should do so to update your record and avoid
problems in the future.
3. A U.S. passport which may be current or expired.
4. A photocopy of your Certificate of Citizenship (N-560 or N-561) or Certificate of Naturalization
(N-550 or N-570). U.S. Citizenship and Immigration Services (USCIS) has informed us that it is
permissible for a school to photocopy these certificates for verification of citizenship.
If you have not taken your Certificate of Citizenship or Certificate of Naturalization to the Social
Security Administration (SSA), you should do so in order to update your record and avoid
problems in the future.
5. An unexpired foreign passport (with MRIV).
IF YOU ARE NOT
A U.S. CITIZEN, you would be considered an eligible noncitizen if your status is one of the
following. Please mark the status that applies to you and provide us with the specified documentation.
U.S. permanent resident. Documentation: Permanent Resident Card (I-551, since 1997) or Resident
Alien Card (I-551, before 1997) or passport stamped “Processed for I-551"
Other eligible noncitizen. Documentation: Arrival-Departure Record (I-94) from the USCIS showing any
one of the following designations: (a) "Refugee," (b) "Asylum Granted," (c) "Indefinite Parole" and/or
"Humanitarian Parole," or (d) "Cuban/Haitian Entrant."
Citizen of the Marshall Islands, Federated States of Micronesia, or Palau. Documentation: Legal
document (passport, birth certificate, etc.) which identifies you as a legal resident of the area.
(Note: Not eligible for federal student loans.)
Please submit one of the forms of documentation listed above to the Office of Financial Aid. We will determine if you meet
the requirements to be considered as a citizen or an eligible non-citizen after we receive the required documentation.
