A Master Promissory Note (MPN or EMPN) must be on file before approved loan proceeds can be disbursed. The parent
borrower may opt (at any time) to complete a PLUS Electronic Master Promissory Note (EMPN) at studentloans.gov
using
their FSA ID (available from fsaid.ed.gov
). Please contact Financial Aid Office at number listed above with questions.

CERTIFICATION: The person signing below certifies that all of the information reported is complete & correct and
authorizes the ISU Office of Financial Aid to obtain all credit checks required for the PLUS. If approved for this loan, they
give Idaho State University permission to use the proceeds of this loan to pay any direct educational charges (tuition,
registration fees, university and housing charges) of student identified above & distribute any remaining funds as indicated.
