LOAN EXIT COUNSELING
Loan Exit Counseling is required of all students who utilize federal loan programs such
as the Federal Direct Student Loan Program (FDSLP). Review and complete the
directions below and, if necessary, return this completed form with attachment to:

Our records indicate that you have left school, dropped to less than half-time enrollment (less than 6
credits for undergraduates or less than 5 credits for graduate students) or will be graduating this year.
Students who have received a Federal Direct Loan are required to complete exit counseling upon
leaving Idaho State University or dropping to less than half-time enrollment (even for one semester).
The fastest and most efficient method of fulfilling the exit counseling requirement is online (using your
US Department of Education FSA ID) at studentloans.gov.
If you do not have Web-access, computer labs are available at the Office of Financial Aid in Pocatello
and at the ISU offices in Idaho Falls, Twin Falls and Meridian.
You are required to either complete the online exit counseling or complete and return the Rights and
Responsibilities Summary Checklist document located at

If you complete the Checklist, please complete this form, attach the Checklist, and return to the
Financial Aid Office using one of the address above.
