provided on your Free Application for Federal Student Aid (FAFSA
). This process is
called verification. Students are required to complete the verification process before the
Office of Financial Aid can establish eligibility for assistance. You must provide the
information requested on this form or you will not be considered for federal financial

INSTRUCTIONS: All financial aid applicants who have recently filed bankruptcy or have a bankruptcy
judgement must provide a copy of the front page of the bankruptcy filing. If you filed a Chapter 13
bankruptcy, attach a letter from the trustee which states that you are allowed to borrow additional
student loans.
