I, together with my Parent On-Record, understand and agree that, as a CONDITIONALLY
ADMITTED student, the following terms and conditions apply to me, over and above regular
academic and student policies:
1.
I must enroll for Term 1 AY 2022-2023 to the degree program offered by De La Salle
University. Otherwise, the conditional admission status is forfeited, and I am no longer eligible
to enroll for the succeeding trimesters.
2.
A Cumulative Grade Point Average (CGPA) of 1.200 or better must be earned at the end of
the first three (3) trimesters.
3.
No academic or non-academic course may be dropped in the first two (2) trimesters.
4.
A Leave of Absence (LOA) cannot be filed in the first term. Any LOA on subsequent terms
within the first three (3) trimesters will extend the conditional admission status by the number
of terms the student is on leave; and
5.
Non-compliance with any of the above conditions will result to my ineligibility to re-enroll for
succeeding trimesters.
It is understood that after compliance with all the above conditions together with the submission
of complete admission credentials (after one trimester), the student shall be released from the
conditional admission status at the end of three (3) trimesters.
