1. Only students admitted to the Marquette University Graduate School of Management (GSM) may enroll in 6995 courses. This two page form will not be processed without all
signatures.
2. After completing this two page form and obtaining the signatures of the instructor and the Chairperson or Director of Graduate Studies of the department offering the course,
forward the form to the GSM. The form will be forwarded to the Office of the Registrar for registration in the course.
