NOTE: This form is not an application for admission; it is required supplementary information.
Form I- 20/DS-2019, which is required for your visa, cannot be issued until you have been admitted to the University
of Florida and you have completed and returned this form to the UF International Center.
• Previous versions of the form (refer to dates above) will not be accepted
• Unsigned forms will not be accepted
• Proof of funds cannot be more than one year old when the I-20 is processed
• Items with (*) next to them must be filled out or the form will not be accepted
Important Instructions:
The university requires proof of financial support for the first year of study: Graduate $50,010;
Undergraduate$48,540. An additional $6,000 is required for your spouse and $2,500 for each child.
These estimates are subject to change and do not include travel expenses. Make copies of the
Certificate of Financial Responsibility form for your records and for use at the United States
Consulate. You should always keep copies of all documents you submit for future reference.
To avoid delays during the I-20/DS-2019 processing, make sure each sponsor completes the
sponsor section of this form. The name must be clearly legible and a signature is required for
each sponsor. If you have additional sponsors, use a new page for each one, including yourself. Be
sure to indicate the dollar amount you will be receiving from your sponsor on the line for the
appropriate source of financial support.
As proof of funding you may attach an official bank statement/letter in English for your sponsor that
clearly states the name of the account holder, the amount available in the account, the currency, the
date, and the bank stamp or seal. Make sure that the account holder’s name appears in English on
the bank statement to avoid delays with your documents. Make sure you keep an original bank
statement to take with you to your visa interview.
Appropriate types of funding include: Savings/Checking accounts, Certificates of Deposit,
Stock Portfolio Investments, Bank Education Loans, Scholarships, Assistantship/Fellowship, and
Government Awards. The following are NOT appropriate types of funds: Statement of
Salary, Credit Cards, Insurance Policies, Property (i.e. Real Estate), and Machinery.
Graduate Assistantship and/or Fellowship:
If you have been awarded an Assistantship/Fellowship from the university, the UF Department
will notify the International Center of the stipend for the Assistantship/Fellowship. The
department does not need to complete this form. Based on the information provided by the UF
Department, an Assistantship/Fellowship might cover part or all of the expenses for the first year and
additional proof of finances may be needed. UFIC will indicate in your New International Student
Request the amount needed if additional funds are required to cover the expenses for the first year at
