How to use this form in a digital workflow

3. The Graduate School Dean will call and convene the Hearing Panel. The Deans representative will forward
copies of this form to all members of the hearing panel. If the hearing must be held online, the Hearing Panel
Chair will use page 4 of this form to send an email of findings and recommended resolution to the student.
Hearing Panel Chair will sign (Step 2 above) and use the button below to submit this form to the Graduate
School.
4. Following the hearing, the Graduate Dean’s representative will duplicate the form and route the form via
email to both relevant parties to the grievance (Step 3 above). Each party may then use the Submit button to
return their form to the Graduate School.
