Purpose: used by College of Arts and Sciences, Communication and Education undergraduate students who wish to design a major or minor not already offered at Marquette.

Student Instructions
1. Complete Sections 1 & 2 of this form using a computer.
a. complete one form for each major and/or minor.
b. a handwritten form will not be accepted.
c. an incomplete form will not be processed and will be returned to you for completion.
2. Print the form using the 'Print Form' button.
3. Sign the form in Section 3; a digital signature is not acceptable.
4. Print your Academic Advisement Report from CheckMarq and submit it with this form as described in Section 2.
5. Attach rationale, description and learning outcomes as determined in consultation with your Individually-Designed Interdisciplinary Major/Minor adviser. College of Arts & Sciences students must provide a letter
of recommendation and signature from faculty member other than the adviser, as indicated in Section 4.
6. Submit the completed form with all the signatures in Section 4 and required attachments to the college of your primary major.
College Instructions
1. In Section 5 enter a decision and a rationale, if denied.
2. Sign with the ImageNow stamp.
3. Scan to the OTR via ImageNow.
4. Notify the student.
