Request for Deadline Extension of Praxis Score Results
Admission to Student Teaching will be granted if students meet all criteria by stated deadlines.
Passing scores for all required Praxis exams must be received in the Office of Education Student
Services by the last day of the semester before student teaching begins.
The following request for deadline extension has been developed for students who have
registered or taken the required Praxis exams but cannot provide a passing score within the
timeframe required.

Reason for Request:
I understand that if extension is approved and passing scores on all Praxis tests required
for licensure are not received by the first day of the student teaching seminar, I will be
administratively withdrawn from both student teaching courses, placements will be
cancelled, and I will have to reapply to student teach the following term.
