INSTRUCTIONS
Form 600-00052: One-Time Registration for Media Individuals
Prior to submitting any application, read the superior court rule and Supreme Court rule regarding
devices and the accompanying administrative order.
Review the different applications and choose the correct form.
Submit application as indicated in the instructions.
1. This form allows media individuals without a permanent registration certificate to obtain permission to
record or transmit court proceedings for a single day or proceeding. See V.R.C.P. 79.2(d)(2)(B); V.R.A.P.
35(d)(2)(B); A.O. 46 § 1(d).
2. Each person requesting one-time registration must complete a separate application.
3. The form must indicate the date or proceeding the applicant seeks to record or transmit.
4. The applicant must sign the form.
5. The form should be presented to court staff. The proceeding will not be stayed pending a decision.
6. The applicant will be notified of a decision.
7. If the application is denied, the applicant can seek review by filing written notice with the Chief
Superior Judge (superior court) or the Supreme Court (Supreme Court).
