Choose the category or categories in which you want to nominate your colleague. If you are
nominating in more than one category, separate entry forms must be completed.
Write an account of no more than 300 words explaining why your colleague should be considered.
An additional account from a colleague or service user / family carer, photographs or video is helpful,
but not essential.
Please make sure that the name of the nominee and the award category is included on all separate
pieces of supporting information.
Complete the entry form and attach it to all other pieces of supporting information.
Please email or post any supporting testimonials and include the name of the nominee
and category.
Closing date for the entry forms is Monday 5th July 2021.

How to Enter
The awards are open to all sta within the CareTech group, including frontline sta as well as
oce based sta across the UK.
All entries are treated confidentially.
Entries will be judged by a panel of CareTech Employees, Service Users & Non-Executive Directors.
Entries will not be returned.
Use one entry form per entry. Photocopies are accepted.
Regional Judging will take place on Monday 23rd August 2021 and Regional Winners announced in
October 2021
National Judging will take place on Monday 20th September 2021 and National Winners announced the
same month.
Rules of Entry
Your nomination needs to include a statement of a minimum of 100 words detailing why you think
the nominee should win.
Please only submit a statement of up to 300 words.
An additional typed statement from a colleague or service user / family carer, photographs or video
is helpful, but not essential.
Tell us in your statement how the person you are nominating upholds the CareTech Values.
Only one person per nomination – we do not accept joint nominations for any individual awards.
For example: for Team Leader of the Year you cannot nominate both John Smith and Sarah Jones
together on one form as only one employee can win. You must nominate them individually. Please
note that any joint nominations we receive will not be counted

Video Entry Option
For service users, residents and students who may have diculty completing a written nomination
on the forms provided, we are introducing the option to submit your nomination via a video this year.
In response to feedback we have received, this video option is also available to sta members that
may experience an issue with completing a written entry.

Please read on for tips to ensure that your video is of the quality required in order to be accepted: -
It is best to have a friend or colleague film you to avoid any issues with heads being cut out or the
video being out of focus.
Please film them as landscape, not portrait – this will look more professional
Film yourself stood against a white or light coloured background
Film in good, preferably natural light, but not too bright
Email videos to awards@caretech-uk.com including the name of the sta member who is being
filmed, their service / home / school / oce and the award they are being nominated for.
Anyone who films and sends a video, must give written permission via email, for it to be shared
on CareTech social media channels and the company website.
Please DO NOT wear a face mask in your video nomination. We need to be able to see your face,
hear you clearly and understand why you are nominating your colleague / carer / teacher.
Make sure you film the video in a safe environment, adhering to Covid-19 rules on social
distancing.

nominated by sta, service users and families.
Support Worker of the Year
For an outstanding performance by a carer. An
individual award nominated by either service
users or sta.
Team Leader / Senior Support
Worker of the Year
For standout leadership by a Team Leader or
Senior Support Worker. An individual award
nominated by either service users or sta.
Team Player of the Year
For exceptional commitment to team success,
reliability and maintaining a collaborative
approach. An individual award nominated by sta,
service users and families.
Team of the Year
For dedication, hard work, consistency and going
above and beyond their expected duties. A team
Award nominated by anyone, including self –
nomination.
Award for Excellence in Oce
Operations
For the most valued Oce Employee,
i.e: Administrator/Payroll/ HR / Finance/
Maintenance/IT/Receptionist. An individual award
nominated by sta, service users and families.
Outstanding Achievement of the
Year
A truly remarkable accomplishment by a sta
member, for the people or person they support.
An individual award nominated by sta, service
users and families.
Manager of the Year
A manager who greatly motivates sta and brings
out the best within their team. An individual
award nominated by sta, service users and
families.
Long Service Award
A sta member who embodies the CareTech
values and has maintained our company’s high
professional standards for 15+ years. An individual
award nominated by sta, service users and
families.
Head of Service, Locality / Regional
Manager of the Year
A manager who provides great leadership to
their sta and has overseen brilliant progress
within their service or area. An individual award
nominated by sta, service users and families.
Social Worker / Therapist / Nurse
of the Year
A clinical specialist who delivers the best
possible care and outcomes. An individual award
nominated by sta, service users and families.
Educator / Trainer of the Year
An educator who champions development and is
passionate about enabling learners to reach their
full potential. An individual award nominated by
sta, students, learners and families.
School/Learning Centre of the Year
A learning facility that has excelled in attaining
successful outcomes for its students or trainees.
A team award nominated by sta, service users,
learners and families.
The Michael Hill Charitable
Community Award
We recognise a generous individual for their
extraordinary community or charity work. An
individual award nominated by sta, service users
and families.

Family Carer / Foster Carer of the Year
A remarkable carer who has consistently gone the
extra mile to significantly improve the life of their
family member or foster child.
Support Worker of the Year
This winner exhibits exemplary conduct and makes a
demonstrable dierence to people’s lives. A Support
Worker who has done something extraordinary for
the person or people they look after.
Team Leader/Senior Support Worker of the Year
A leader or senior sta member who makes a
tangible and on-going dierence to people’s lives,
while motivating their team to do the same. An
enthusiastic carer who takes a truly person-centred
approach to the residents they look after.
Team Player of the Year
The winner of this accolade puts the team first,
communicates well and encourages others to work
to the best of their abilities. This individual sees the
bigger picture of the collective goals the team needs
to meet and strives to achieve them. A valued sta
member whom colleagues look to for inspiration.
Team of the Year
A cohesive team that has achieved extraordinary
results for those they support, working eectively
together for the benefit of their service users,
residents, students or trainees. These worthy winners
are recognised by the rest of the company as a
model example of providing support and consistently
delivering high standards of care, education or oce
support.
Award for Excellence in Oce Operations
A valued employee who works within the group in
an admin role; either in a service, regional oces or
at a shared service centre. This winner is recognised
by others as continually going out of their way to
support frontline services and contributes to good
outcomes for the people we support.
Outstanding Achievement of the Year
An enthusiastic and dedicated carer who has gone
beyond the call of duty to make a profound impact
on the life of a service user, patient, resident or
student.
Manager of the Year
A manager who is a beacon of encouragement to
service users, families, colleagues and their peers;
an inspiring leader who models great practice and
thus ensures excellent outcomes for the people they
support.
Long Service Award (15+ years)
This award recognises an employee with over 15
years service who has consistently made a dierence
to people’s lives. This winner is universally respected
by colleagues and peers as always working for the
good of everyone and the wider company.
Head of Service, Locality / Regional Manager of the
Year
A manager with exceptional leadership skills who
has been inspirational within their service, locality or
regional area. This individual promotes best practice
to their teams and supports them to deliver tailored
care to their residents.
Social Worker / Therapist/ Nurse of the Year
This winner is passionate about service delivery, using
creative approaches to engage and motivate service
users, patients, children and young people. They
work collaboratively to achieve the best possible
outcomes and ensure the well-being of those they
care for is their highest priority.
Educator/Trainer of the Year
A devoted educator who has delivered excellent
results and seeks to ensure students and trainees
enjoy the process of learning. This teacher or
trainer will be open to new teaching methods and
dedicated to propelling learners to the height of their
capabilities.
School / Learning Centre of the Year
An learning or education facility that has excelled in
achieving positive results for its students or trainees.
The winning sta team are innovative and committed
to the delivery of learning outcomes and excel at
meeting the educational and developmental needs of
their students or trainees.
The Michael Hill Charity Community Award
This award recognises a selfless sta member who
goes above and beyond to improve the lives of
others outside of their working day. It rewards their
outstanding community or charity work and their
dedication to worthy causes.
