Secretary is also available to answer any questions you may have regarding this
certification.

including the provisions thereof regarding actual or potential conflicts
of interest. While serving the City these past 12 months (or portion
thereof if newly elected or appointed), I confirm that I have followed
all provisions of the Ethics Code.
