PLUS Loan due to an active bankruptcy may submit documentation along with this form for the student to be considered
for an additional Federal Direct Unsubsidized Loan.
INSTRUCTIONS

Attach documentation from the United States Bankruptcy Court. Documentation must contain the followin

Date of order confirming the bankruptcy plan
The number of months that the debtor is scheduled to make payments
Statement that the debtor shall not incur additional, non-emergency, consumer debt (in excess of a
certain amount) without written approval of the Court or the Trustee
