Please note:
1. An I-20 cannot be issued without this form and the supporting documentation.
2. If you have more than one sponsor, each one must sign an Affidavit of Financial Support. Use the
additional Affidavits on Page 2 when you have multiple sponsors.
3. If you are going to live with a local host who is a U.S. citizen or permanent resident, you can subtract
the expense for Room and Board from the total amount of funding. In this case, please also submit a
letter from the host alongside with a copy of their ID.
4. Please keep copies of all financial documents, as you will need them to show proof of funding to
U.S. Consular officials when you apply for a visa and when you enter the United States.
