Your FAFSA application was selected by the U.S. Dept. of Education for review in a process called “verification.” In this process we are
required by federal law (34 CFR, Part 668) to compare the information from your FAFSA application with the information provided on
this form. If there are differences between your application information and your verification documents, we may need to reprocess
your FAFSA information. We cannot process your financial aid until verification has been completed. Please provide the required
documents as soon as possible.
This form cannot be photocopied or scanned. This form needs to be mailed in with original signatures and notary stamp.

You must sign in the presence of a notary. You will need to present an unexpired valid government-issued citizenship
documentation such as, but not limited to, U.S. Passport, birth certification, a Certificate of Naturalization or a Certificate of
Citizenship. The institution will maintain a copy of your photo ID that is annotated with the date it was received and
reviewed, and the name of the official at the institution authorized to receive and review the student’s ID.
