INCOMPLETE APPEAL PACKAGES WILL NOT BE ACCEPTED
Students who believe there were exceptional circumstances preventing them from making
progress must complete this form and attach all required documentation (see page 2 of this form
for details).
Note: Students awaiting an appeal decision will need to make payment arrangements for
existing enrollment. Before submitting your appeal, you must have a current FAFSA and
all the required documents on file in the Financial Aid Office, and you must be registered
in coursework directly from your current declared degree.

GPA

point average of less than a 2.0
(a C average).
Grade point values are A = 4.0,
B = 3.0, C = 2.0, D = 1.0, F = 0. In the
case of a repeat course, both grades will
appear on the transcript but only the
last grade posted for the repeat course
is calculated in the cumulative GPA.
Completion Rate
You have not completed at least
67 percent of your attempted
course work.
Completion rate is calculated by
dividing completed coursework by all
attempted hours. The completion rate
calculation includes the following:
Withdraw (W), fail (F), incomplete (I),
not reported (NR), Progress (PR) and
audit (AU) and transfer hours. It also
includes hours for which a student did
not receive financial aid.
Maximum Time Frame
You have exceeded the number
of attempted hours required to
complete your program of study
by approximately 20%.
All attempted college level hours are
included in the Maximum Timeframe
Calculation. Preparatory coursework
is not used to calculate attempted hours.

All Appeals must include an Official Degree Plan signed by an advisor. You can get an official
degree plan by meeting with the department advisor in your school of study, or an advisor in the
Welcome and Advising Center (WAC). NOTE: ALL COURSE SUBSTITUTIONS MUST
BE APPROVED IN WRITING BY THE DEPARTMENT CHAIR.
Required Documents for GPA and Completion Rate Suspension:
Carefully follow the instructions below. Check boxes have been provided to help you confirm
that each step is complete.
Personal Statement
The personal statement is a typed letter providing a detailed explanation of:
The exceptional circumstances that have prevented you from making satisfactory academic progress.
Detailed information on what has changed AND what measures you have taken to ensure progress in the
future.
Your educational goals.
If your transcript indicates difficulty for more than one semester, the petition must address the circumstances
pertaining to each semester and include an explanation of how the circumstances have been resolved.
Supporting Documentation
You MUST provide documentation in support of your appeal. If you choose not to provide
backup documentation, your appeal may be denied and the denial will be final.
Appropriate documentation might include:
A physician’s statement, medical documentation, death notice, legal documentation, etc.
Any letter of support written by third parties DIRECTLY related to the situation. The letter should include a
name, address, phone number, and relationship to student.
Any other relevant documentation that provides confirmation of the circumstances mentioned in the personal
statement.
A degree plan approved and signed by an academic advisor
Must be enrolled in courses directly from your degree plan for the semester that you
are requesting reinstatement of aid.
Required Documents for Maximum Timeframe (MTF) Suspension:

MTF, when you plan to graduate, and how you plan to finish your degree successfully.
A degree plan approved and signed by an academic advisor
Must be enrolled in courses directly from your degree plan for the semester that you
are requesting reinstatement of aid.
