Submitting a petition does not guarantee it will be approved. The petition process requires extensive review by the Committee. All
transcripts from previously colleges attended must be on file with Enrollment Services. If you have a hold on your account and/or the
petition is incomplete it will be returned unprocessed to the address on file.

Extenuating Circumstances: Students ineligible for financial aid based on extenuating circumstances, something beyond the student’s
control that specifically prohibited the student from meeting SAP standards. Extenuating circumstances must be supported by official
documentation.
Examples of extenuating circumstances:
Death of an immediate family member
Documented medical condition or serious illness, major accident, or injury.
Victim of a crime, unexpected disaster, or other emergency circumstances
Maximum Timeframe Appeal Extension: Students ineligible for financial aid based only on excessive units must explain the reason for
the excessive units.
Examples of reason for excessive units are:
Completed ESL or Basic skills courses
Change in major (one time only)
Education goal requires completion beyond 90 units for AA/AS Degree or 45 units for Certificate.
Requesting change from Certificate to AA/AS
Instructions:
1. Complete this form
2. Attach a signed, detailed personal statement, which describes:
The extenuating circumstances and/or the reasons you have exceeded the unit maximum.
The statement must include steps you have taken to ensure you will make satisfactory progress in the future and/or complete
your educational goal
3. Attach photocopies of all relevant documents, (such as doctor’s letter to support medical claims, police reports, or death
certificate.) Make sure there are dates on any documentation provided.
4. Attach the email confirmation page for verification that you completed the SAP Counseling Session “The Essentials to
Understanding Satisfactory Academic Progress (SAP) and what is Means to youat http://msjc.get-counseling.com
.
5. Schedule an appointment with an academic counselor. Attach a current Education Plan signed by an academic counselor which
outlines the required courses for your program of study and expected date of graduation or transfer.
6. Make sure to clear all holds and/or outstanding debts to the college.
7. Make sure all transcripts from previously attended colleges are on file with Enrollment Services.
8. Submit the completed petition and all attachments to the Financial Aid Office.
I understand if my petition is approved, I agree to the terms of this petition. If my petition is approved I will be required to follow the
courses outlined in my Education Plan and will only receive financial aid for the approved SAP courses listed. I understand that failure to
follow my Education Plan will result in my petition being revoked and no further petitions or revisions will be allowed. I certify that all
information provided is true and correct to the best of my knowledge. I understand the Committee’s decision is final. I also understand
if my petition is approved my initial disbursement of funds will not occur until my file is complete and this may take 4 to 6 weeks.
