2019-2020 FINANCIAL AID SUSPENSION APPEAL FORM
IMPORTANT: All appeals must be received in the Office of Financial Aid no later than the following 2019-2020 Appeal Deadlines:
Fall 2019 Thursday 6/20/19
Winter 2020 Friday 11/22/19 (for students on approved Fall 2019 appeal: Friday 1/17/20)
Spring/Summer 2020 Friday 4/3/20 (for students on approved Winter 2020 appeal: Friday 5/22/20)
Incomplete appeals will be denied. Late appeals will be considered for the following semester only.

Login to the National Student Loan Data System (NSLDS) and provide the following (see attached form for instructions):
If you have received Federal Pell Grant while in college, list your current Pell Lifetime Eligibility Used (LEU):
If you have never received Federal Pell Grant, indicate N/A.
Print a copy of your NSLDS Loan History and attach to this appeal. If you currently owe on
any Federal Direct Student Loans, indicate your Loan Servicer(s) in the box to the right.
If your loan balance is $0 or you have never received federal student loans, indicate N/A.
If you are currently on Financial Aid Suspension, you may appeal to have your eligibility reconsidered. The appeal process is an opportunity to
present your reasons for poor academic performance. The submission of an appeal does not guarantee the reinstatement of financial aid. Rather,
a committee will review your appeal and you will be notified whether or not the appeal has been approved. In order for an appeal to be considered,
a student must have met the conditions of any prior appeals. Exceptions to this will only be made on a case-by-case basis.
Directions: Complete this appeal form as well as all other required documentation listed below. You may submit this form, along with all other
required documentation, by paper-clipping all items together and returning them to any drop box on the HFC campus or by mail or fax to the
address and fax number listed above. Appeals must be submitted by the appropriate semester deadline.

Typed Statement of Appeal--On a separate sheet of paper you must TYPE a Statement of Appeal that includes the following:
A. State the reasons why you did not make Satisfactory Academic Progress (SAP) as well as what has changed that will
allow you to improve your academic progress.
The inclusion of neutral third-party documentation that supports your Statement of Appeal is strongly encouraged.
B. If you are appealing because you have attempted more than 150% of the credit hours required for your current program of
study, please also discuss the actions you have taken or will take to complete the program within the timeframe allowed. If
you are attempting a second degree/certificate or if you have changed your program of study, please discuss the reasons
you are requesting federal financial aid to complete an additional degree/certificate or for the additional credit hours
required of a program change.
2019-2020 Financial Aid Academic Plan (150% Rule Violators only)
If you have been notified that you have attempted more than 150% of the credit hours required for your current program of study,
in addition to the above you must also complete and submit a “2019-2020 Financial Aid Academic Plan for Satisfactory Academic
Progress ‘150% Rule’ Violators” form to the HFC Office of Financial Aid along with this form and all other appeal documentation.
