TRANSMITTAL FORM FOR DONORS 2019-2020
Please find enclosed ______ cheque(s) payable to Tyndale University College & Seminary in support of the
FORGIVABLE LOAN PROGRAM (herein known as FLP) as an irrevocable donation(s) from the following
donor(s), to whom please issue an appropriate receipt.
I/we understand and acknowledge that this donation is to be used for supporting Tyndale students with
financial needs who have enrolled in the FLP. I/We understand that my immediate family members are not
eligible for FLP funding. Direction and distribution of these donated funds is made solely at the discretion of
the Tyndale Financial Aid Office.
DONORS: Please do not place the student’s name on the cheque, only memo that the donation is for FLP.

*Monthly donation will be stopped by the end of the academic year. A new form has to be filled out for a new academic year.

Tyndale will issue a charitable donation receipt for 100% of the donation. 90% of the donation will be
distributed to student need. 10% will go toward administering the Fund.
