Grades that were previously "Incomplete" and are now complete, are NOT considered to be a grade reported
in error or reported late. Please do NOT complete this form for an Incomplete grade being updated.
The computer program that determines whether you were in compliance with the Financial Aid Progress Standards
was run after grades were posted.
The assessment of whether you were in compliance was based upon the grade(s)
that were submitted and recorded on
the college computer system as of that date.
However, we realize grades may have changed since then. Therefore, if you would like your records reviewed to see
if you are now in compliance and can be removed from financial aid suspension, please print and fill out this form,
and then return it to the Financial Aid Office within ten days of receiving your changed grade.
* If a grade of Incomplete is what placed you on suspension, you must
submit the final grade along with this
form to the Financial Aid Office for review.*

Students are required to maintain a 2.0 cumulative grade point average.
Students must complete 67% of the cumulative attempted credit hours.
Students must complete a degree within 150% of the published length of the program measured in credit
hours attempted.
You will be notified of the results of your request within two weeks of turning in your form.
