2020-2021 150% Warning Review Request
You have received a 150% Warning email that you are reaching 150% of the published required length of
your program in cumulative credits attempted. Hawkeye is a 2-year community college and strives to
enable students to graduate with their degrees with the ability to transfer on to a 4-year institution. Federal
Regulations require Hawkeye to monitor students’ progression towards completion of a degree.
Remember, you are still eligible to receive your financial aid for the current term. However, you will be
placed on Financial Aid Suspension at the end of the term.
If you will be graduating and/or transferring, at this time your financial aid status will not transfer.
However, if you choose to return to Hawkeye, you will be returning on financial aid suspension and will
lose your eligibility to receive financial aid in future semesters.

Provide an explanation of the circumstances that prevented you from meeting the 150% standards
(ie. Completion of multiple programs). (Use separate sheet of paper if necessary.)
Attach a completed Academic Planning Worksheet. It is very important that this plan is outlined
through expected term of graduation so the reviewing committee can evaluate the required courses and
semesters needed in your plan. You will need to schedule an appointment with your Success Specialist
to fill this out. You should print off your Academic Evaluation (Degree Audit) from your MyHawkeye/
WebAdvisor/Academic Planning account and be ready to discuss the classes you have left to take with
your academic advisor.
I understand that this request does not guarantee approval. This review is to avoid being placed on financial aid suspension due
to not meeting the 150% standards at the end of the term. The academic plan provided shows my plan for graduation and only
required classes for me to graduate at Hawkeye Community College.
