REFERENCE FORM
Instructions: Fill out the personal information below and have the bottom portion completed by your references. Two references
are required if applying for Academic Senate or Rose Cortez Memorial Scholarship: One must be from an FRC faculty or staff
member (be sure to follow the reference criteria of the scholarship for which you are applying). If you have not yet attended FRC,
one reference must be completed by an instructor at your high school or previous college. The second reference must be from a
personal reference outside of FRC or per the criteria of the scholarship for which you are applying (e.g., high school teacher or
administrator, employer, clergyman, community organizer, etc.). Letters from personal friends or relatives are not acceptable. You
may use the attached form, or you may submit letters of recommendation that must answer all the questions contained in the reference
form. Letters of recommendation must be attached to your application with your FRC student ID number on it; do not submit sealed
envelopes. Attach additional sheet if necessary.
