International applicants who plan to attend Bradley University on an F-1 student visa must complete
this form in full before an I-20 Certificate of Eligibility will be issued. An uploaded copy of the
document to the Application Status Page is acceptable. The form must be shown to the U.S. consulate
during your visa interview and be available to show to the customs officer at the U.S. border.
