COVID-19 INFORMATION
Use this form if you have received a federal exemption to COVID-19 isolation requirements for entry into
Canada. You must qualify under the essential service worker group exemption or have been issued a
federal exemption before you receive a provincial one. For more information on restrictions, please visit
alberta.ca/covid-19-travel-advice.aspx.
Please submit your request to BizConnect@gov.ab.ca with the subject line ‘Attention: Exemption Team.’ It can
take up to 10 business days for your information to be reviewed. You may be contacted with questions or if any
additional information is needed.

Rationale for classifying as essential service workers:
Attach your corporate COVID-19 plan describing precautions to keep workers safe. If you do not have one, a
template is available at: https://www.alberta.ca/guidance-documents.aspx#toc-2
Optional: attach the designated accommodation COVID-19 plan (hotel’s COVID-19 policy or a letter stating
how they will support your employees):
Travel arrangements to and from worksite, and to and from the airport/land border:
- Provide a plan that may include travel requirements such as, but not limited to:
- Mask wearing while traveling or when unable to social distance
- Frequent hand hygiene
- Attempting to social distance at all times
- Use pay at the pump to obtain gas
- Obtain food at drive through locations and
- Contactless car rentals, taxi/rideshare (nd guidance here: https://open.alberta.ca/publications/

- Do not use public transit
