In order to qualify for this rebate, the model # must be listed on the ENERGY STAR
website at https://www.energystar.gov/productfinder/product/
certified-connected-thermostats/results. By signing below, the purchaser certifies that
the smart thermostats for which they are claiming a rebate is installed at the address
listed above and authorizes Cleco to perform a phone survey or physical inspection to
confirm installation.
All rebate forms must be received within 45 days of purchase of your new smart
thermostat and must include a customer service invoice and photo of the installed Smart
Thermostat to be eligible for the incentive. Please allow 4-6 weeks for processing. For
