Employee Exit Checklist
When an employee separates from employment at Orange County Community College,
it is important to ensure an orderly transition, which includes the protection of the
College’s assets.
This form has been developed to help SUNY Orange management complete all
necessary separation actions.

Collect from employee:
______ Office and/or desk keys (Return office keys to Security Department)
______ Swipe Card (Return to Security Department)
______ ID Badge (Return to Security Department)
______ Equipment (Laptop, cell phone, tools, radio, etc.)
______ Parking Permit (Return to Student Activities)
______ Retrieve voice mail password from employee and change name on voice
mail message
______ Make sure employee deletes personal files from desktop computer and
server
