Electronic Thesis and Dissertation (ETD) Access Form
The PSU Library needs your FULL name for cataloging purposes.
PSU may need to contact you months from now regarding your ETD submission. Please think carefully
about long-term contact information.
You control the copyright to your ETD. If you have used others’ copyrighted materials or entered into
special agreements, you may not own the entire copyright to your ETD. This ETD agreement assumes
you have sufficient rights to grant PSU non-exclusive rights to make your ETD publicly accessible.
By completing and signing this form, you hereby grant Portland State University non-exclusive rights for
the term of the copyright to make publicly accessible and archive your ETD. PSU currently makes ETDs
available through PDXScholar. However, the name and/or format of the tool used to make ETDs
available could change in the future. This agreement grants non-exclusive rights to make publicly
accessible your ETD in all forms of media, now or hereafter known. This agreement is independent of
the publishing agreement you make with ProQuest during the ETD submission process.
Select only one of the following three options: A, B, or C.
A. Open Access: I request that the PSU Library immediately release the full text of my ETD for
unrestricted access (i.e., available to anyone via the Internet) through the PDXScholar or other systems.
OR
B. Open Access with Delayed Release: I request that PSU embargo (block) access to the full text of my
ETD for the period of time indicated below. After the designated time, my ETD will revert to Open
Access. I understand that even with Delayed Release, my abstract will be publically available via the
Internet.

OR
C. Campus Restriction: I request that the PSU Library immediately release the full text of my ETD to
the PSU campus only (i.e., requires Odin login to access). I understand that even with Campus
Restriction, my abstract will be publically available via the Internet.
You must have permission from the Office of Graduate Studies to choose Campus Restriction. Attach proof of OGS
approval.
If you choose option B above, you MUST select one of the two options below.
1. I request that PSU embargo (block) access to my ETD through the PSU Library for one year
from the date my manuscript is approved by OGS.
You may choose a one-year embargo at your discretion.
2. I request that PSU embargo (block) access to my ETD through the PSU Library for two years
from the date my manuscript is approved by OGS.
You must have permission from the Office of Graduate Studies to choose a two-year embargo. Attach proof of OGS
approval.
