EVENT REQUEST FORM
All student organization events must be approved by the Student Life Office. In order to obtain approval,
Event Request forms for each event must be completed and submitted to the Student Life Office
TWO WEEKS prior to the event.
Any Event Request Form submitted less than two weeks prior will not be approved.
