1. You should only complete this form if you wish to make a cash donation to the Donations and Bequests
Account
for the purpose of reducing the National Debt.
2. Please send completed forms to CRND@dmo.gov.uk. Cash donations are to be sent via electronic payment. The
bank account details will only be confirmed following receipt of a correctly presented donation form.
3. Once the donation proceeds have been received, it will not be possible to obtain either a full or partial refund of
your donation.

By checking this box, I confirm that:
a) I wish to make a donation to reduce the National Debt.
b) I understand that I cannot request either a full or partial refund of my donation once the proceeds have been
received by the Commissioners for the Reduction of the National Debt.
c) The monies being donated are my own to give, and not the proceeds of crime, money laundering or other
illegal activity.
d) I understand that any personal data provided by me will be held by the UK Debt Management Office in
accordance with the relevant legislation.
