If a party is represented by counsel, the certification may be filled out by counsel as follows:
I, __________________________, as the lead counsel for ______________________, a complainant or respond-
ent in an investigation under section 337 of the Tariff Act of 1930 before the U.S. International Trade Commission,
attest that I have received the materials sent by the Commission regarding the 337 Mediation Program and dis-
cussed them with the party under my representation.

If a party is not represented by counsel, the certification shall be filled out by a representative of the party (.i.e. a
business principal) as follows:
I, __________________________, a representative of _______________________, a complainant or respondent in
an investigation under Section 337 of the Tariff Act of 1930 before the U.S. International Trade Commission attest
