I, the above defendant in this case hereby make my motion for deferred disposition in this
matter. In connection with this motion, I enter a plea of:

I hereby waive my right to a trial and waive my right to a jury trial. I also agree to provide proof of
current financial responsibility as a condition of my deferred disposition for any and all “Failure to
Maintain Financial Responsibility” violations.
I understand that if this motion is granted the Court will defer final disposition for a period of ninety
(90) days subject to the conditions set forth in the order below.
I authorize the City of Bedford Municipal Court to forfeit this cash bond, and apply it to the above
referenced cause to pay the special expense owed for deferred disposition.

I am required to complete a driver’s safety course and return the completion certificate to the
court no later than 60 days from the date of this form. Failure to provide proof of completion
of the driver’s safety course will result in a conviction of this offense being reported to the
Department of Public Safety and will reflect on your driving record. Defendant shall not be
subsequently convicted during the deferral period of any Class C misdemeanor.
