• I accept the offer of admission to the Professional Doctor of Physical Therapy program at Concordia University, St Paul.
• I understand that this offer is contingent on the requirements that I may have yet to fulfill. Failure to return this signed form prior
to the start of classes will forfeit your spot in the upcoming cohort.
• I understand that in accepting this offer of admission, I am accepting the responsibility for paying travel and lodging costs associated
with clinical rotations. I understand that although a large percentage of clinical facilities are located within Minnesota, I may be
assigned to a clinical education experience outside of the state. I understand that I can expect to be assigned to at least one clinical
education experience that is greater than one hour from St. Paul. I also understand that I will not be assigned to a clinical education
experience in the setting in which I am currently or have previously been employed or to which I have a formal work commitment.
• I authorize Concordia University, St. Paul to register me for courses scheduled in accordance to my cohort schedule.
• I accept responsibility to have knowledge of registration, withdrawal, and refund policy procedures set forth in the current Concordia
University, St. Paul academic catalog and calendar.
• I understand that in order to change my course schedule, including dropping or adding courses, I may do so through consultation
with my advisor or through CSPConnect. This must occur prior to the add/drop deadline.
• I accept responsibility for payment of all charges relating to the registration of all courses listed to complete my program, including
penalties and collection fees should my account become delinquent.
• I understand that if my balance is not paid I will be placed on hold status, which would prevent me from registering for future terms
and obtaining a copy of my academic transcript.
