DIRECTIONS FOR FILING FORM FI-17“REPORT OF
UNAUTHORIZED USE OF LICENSE/REGISTRATION”
Please read all of the following directions CAREFULLY BEFORE filing an FI-17 with DMV.
DMV’S DIVISION OF FIELD INVESTIGATION HANDLES COMPLAINTS REGARDING
THE MISUSE OF DOCUMENTS ISSUED BY NYSDMV.
1. If your driver’s license or non-driver ID card has been used to commit a crime such as fraudulent credit card
use, or to fraudulently obtain bank checks and passports, you should first:
� file a report with your local law enforcement agency.
� notify your bank or other financial institutions.
� keep a record or journal of your actions regarding the loss (for example, any police agency you reported the loss
to, detectives who may be working on the reported loss, etc.).

submit a Report of Unauthorized Use of License/Registration (FI-17) with all pertinent evidence that you
have collected concerning the alleged criminal activity.
2. If you have been contacted about a registration that you do not know anything about:
� submit a Report of Unauthorized Use of License/Registration (FI-17) to the Division of Field Investigation of DMV.
� if outstanding parking tickets are involved, you may want to request a hearing by contacting the Parking
Violations Bureau that issued the notice to you.
Any evidence that has been gathered from the above actions should be forwarded to us when you file your Report of
Unauthorized Use of License/Registration (FI-17).
OTHER IDENTITY ISSUES SHOULD BE HANDLED AS EXPLAINED BELOW.
1. If you are disputing traffic tickets issued in your name:
� please answer the outstanding tickets in the appropriate court (if the tickets are dismissed, the dismissals will
provide evidence needed to proceed with a possible investigation).
� if you know who committed the offenses you are being charged with, file criminal charges against them with
your local law enforcement agency.
2. If you have been notified that your license is being suspended because of “Default Convictions” on your record:
� please contact DMV’s Traffic Violations Bureau at (518) 474-0941; NYC and Long Island motorists should
call (718) 488-5710.
� ask for an “Application to Reopen Default Convictions” (DMV Form AA-3.3). This should be filed through
DMV’s Traffic Violations Bureau. (If the tickets are dismissed, they may also be used as evidence for any
investigation we conduct.)
3. If you have been notified about an accident in which you were NOT involved:
� you may request a copy of the Police Accident Report (DMVForm MV-198C). The fee for this report is $10
(search fee) plus $15 per copy. Please return to the address listed on form.
� you may request a hearing (non-involvement only) through DMV’s Certified Document Center by calling 518-474-5630.
If, as a result of the above actions, you discover that your NYS DMV documents have been used fraudulently, please
send an FI-17 along with any evidence you have.
