Cross-Registration and Off-Campus Study Approval Form
Matriculated students at SUNY Plattsburgh must obtain campus approval prior to taking courses hosted at another institution in order to evaluate transferability
and applicability to degree requirements. SUNY Plattsburgh tuition may be applied to courses which are essential for on-time graduation and are taken at another
SUNY institution. Alternatively, students may choose to take courses at other institutions that are not essential for on-time graduation, but they will be responsible
to pay directly for all applicable tuition and fees. For more information on Cross-Registration and Off-Campus Study, including important information regarding

Instructions:
• Fill in the student information and the first two columns of the course information on this form.
• Transfer course equivalencies may be found at https://www.plattsburgh.edu/admissions/transfer/transferring-credit/equivalencies.html. Requests for
courses not on the table must be accompanied by a copy of the course description.
• Consult with your Advisor and Chairperson for completion of the remaining sections of this form and approval signatures.
• Requests to take a general education course must also be signed by the Associate Vice President for Academic Affairs, Kehoe 807.
• After receipt of this form, the Registrar’s Office will email you at your plattsburgh.edu account to notify you whether the course is eligible for cross-
registration. The email will include further instructions to fill out the SUNY online application that is required in those cases. If you do not receive an email,
please contact us at registrar@plattsburgh.edu to confirm receipt of this form.
