INSTRUCTIONS FOR COURSE SUBSTITUTION/WAIVER REQUEST
1. The academic/faculty advisor should initiate this request as a part of developing the student’s
program completion plan.
2. The advisor completes the following steps:
a. Completes the student and course information sections.
b. Obtains the necessary department chair/program director approval.
c. Obtains the dean’s approval, if required.
d. If approved, records the substitution/waiver in the degree audit system.
e. Sends an email notification to the student regarding the outcome of the request.
f. Submits the form and any supporting documentation to the Office of Records and
Registration.
For most substitutions/waivers, the approving department chair/program director or dean is from
the school/department that sponsors the course. The Undergraduate Programs office will review all
general education requirements.

The student’s academic advisor should send a written response to this petition to the student’s
current e-mail address within 20 working days of the receipt of the petition.
