information will be managed and to inform them of their rights. But more than that, we want to be transparent,
open and ethical in our practices.

For individuals under the age of 16 to join ACCA (or under the age of 18 in Kenya) we require a parent, guardian
or individual with parental responsibility to complete this form providing consent for ACCA to collect and use the
applicant’s personal data.
