Task: Following the steps listed in the procedure, complete the patient intake, recognizing nonverbal
communication and possible barriers to communicating with the patient.
Supplies & Conditions: Patient’s chart, pen (if paper chart), computer (if EHR). You need to elicit information about the
patient’s chief complaint for the provider. Additionally, you notice in the chart that today is the
patient’s birthday. Optional: Procedure 5–1 form (progress note) can be used to complete this
activity.
Standards: A maximum of three attempts may be used to complete the task. The time limit for each attempt
is 10 minutes, with a minimum score of 70 percent. Scoring: Determine student’s score by
dividing points awarded by total points possible and multiplying results by 100.
Forms: Procedure 5–1 Scenario and Procedure 5–1 Form (optional). Procedure forms can be
downloaded from the Student Companion website; procedure scenarios are provided in the
Instructor’s Manual.
