Task: To determine near distance visual acuity of a patient by using the Jaeger system.
Supplies and Conditions: Jaeger near vision acuity chart, patient chart/EHR, and a pen.
Standards: A maximum of three attempts may be used to complete the task. The time limit for each attempt
is 10 minutes, with a minimum score of 70 percent. Scoring: Determine student’s score by
dividing points awarded by total points possible and multiplying results by 100.

1. Identify the patient, introduce yourself, and explain
the rationale for performance of the procedure. Show
awareness of the patient’s concerns related to the

2. Have the patient sit up straight but comfortably in a
well-lighted area.

3. Hand the Jaeger chart to the patient to hold, between
14 inches and 16 inches from the eyes.

4. Instruct the patient to read (out loud to you) the
various paragraphs of the card with both eyes open,
first without wearing corrective lenses and then with.

5. Record the results and problems, if any, on the
patient’s chart to assist the provider in determining the
visual acuity of the patient.
