Please note:
• You can lodge this application at a Department of Transport
and Main Roads (TMR) Customer Service Centre, or if
you live in a rural area, a Queensland Government Agency
Program Ofce or police station that conducts vehicle
registration business.
• A self-assessment inspection of the change of vehicle details
must be carried out by the registered operator/representative
of the registered operator, a departmental approved agent
or authorised motor dealer prior to lodging this application.

• A Certicate of Modication or an Approval letter may need
to be provided if the vehicle has been modied in any way
from its original design.
• TMR must be notied if a heavy vehicle is used in a
combination where registration fees or vehicle use are
different to current fees/use.
• You must provide evidence of identity (EOI) at the time of
making this application. For example, original Queensland
Driver Licence. For EOI requirements refer to the separate
Evidence of Identity Requirements for Individuals and
