College of Human Sciences & Education (CHSE) students wishing to declare or adjust their intended date of graduation
can do so through this form if the adjustment cannot be made via their myLSU account. The form can be submitted to
CHSE in one of the following ways:
1. To the CHSE Office of Student Services, 236 Peabody Hall, between 8:00 AM and 4:30 PM, Monday – Friday
2. Via email to chseinfo@lsu.edu or to a CHSE counselor/advisor
3. Via fax to 225/578-3613.
This form will be processed within 7 to 10 workings days of submission. Students will be notified by email once the request
has been completed.
If a student has already completed graduation checkout, he/she may be required to complete new graduation checkout pa-
perwork. Students will be contacted by the CHSE Office of Student Services regarding changes if needed.
