(Please refer to the Program Codeslisted on the second page of this form.)
I understand my request for a change of program/major will be evaluated based on the following criteria:
1. I am in good academic standing: 3.00 program GPA for graduate students; 2.00 cumulative GPA for undergraduate students.
2. If I am on academic probation, and the dean of the school that offers my desired program determines that I satisfy the program’s
admission requirements, my request will be forwarded to the Committee for Admissions and Academic Probation for review and approval.
3. I have satisfied all admission requirements for the desired degree program (e.g., if I desire to change to an MBA program, I will be
required to submit a GMAT/GRE score unless I am eligible for a waiver).
4. I must have an active degree program as of the date of this request. Note: Students who have been stopped-out from their degree
programs (due to non-attendance for three consecutive trimesters) should contact the Admissions Office at applications@ggu.edu for
guidance.
5. If I am an international student (i.e., F-1 or J-1 Visa), I must be in good immigration status and be in compliance with all visa
requirements. Note: Students will be issued new I-20 forms if their requests are approved.
I understand that if my change of major/program is approved, I become subject to the program requirements that are in effect on the date the
request is approved (i.e., current catalog), not the requirements that were in effect when I was admitted to my former degree program.

DECLARING GRADUATE PROGRAM CONCENTRATIONS
Students may declare concentrations when they have completed the coursework
required for their desired concentrations, or after the “Last Day to Drop Course
without Tuition Charge” (per the Academic Calendar) for their final terms, if they
will be able to complete their concentrations in their final terms.
To be eligible to declare concentrations, students must have already completed
the required concentration coursework, or be able to complete it in their final
terms without requiring waivers, substitutions, or directed study courses, unless
they are approved in advance by the department chair, program director, or dean.
Students may declare up to two concentrations in a given degree program.
Students seeking to declare more than two concentrations will be required to
appeal to the dean or vice president of academic affairs for approval. Students’
diplomas will list the concentrations that they had successfully completed at the
time their degrees are conferred. Students may not declare additional
concentrations after their degrees have been conferred.
