Parent Plan for Seeking Employment
Documentation of seeking employment shall include a written parental declaration signed under
penalty of perjury stating that the parent is seeking employment. The declaration shall include the
parent’s plan to secure, change, or increase employment and shall identify a general description of
when services will be necessary. Employment-seeking activities may include, but are not limited to:
conducting internet searches of potential employers, contacts with farm contractors, meetings with job
counselors, preparing a resume, calling on or visiting potential employers, and conducting job
interviews.
Describe below your plan for your employment search activities, include the dates and times you will
need childcare services in order to perform these activities. Please note that the law does not allow
child care services to be provided more than five days in a week or for more than 30 hours in a week
when the need for services is due to employment search activities.
