under Schedule 3 Part 2 Paragraph 8(2) of the Act then the person whose name appears first in the proprietorship register, i.e. the first
named person set out in the panels below, will be entitled to be entered in the register of members of the association under Schedule 3
Part 2 Paragraph 8(4) of the Act.

other addresses can be any combination of a postal address, a box number at a UK document exchange or an electronic address. For a
company include the company’s registered number if any. For Scottish companies, use an SC prefix and for limited liability partnerships,
