Financial Aid Award (i.e.: request work study, decline loans, decline Financial Aid if you will not attend RCTC for the
entire school year, etc.). If none of these items apply, you DO NOT need to submit this form.
NOTE: If you do not plan to attend RCTC for the entire school year, YOU must drop your own courses. Your courses
will NOT be dropped for you based on the completion of this form.
