I intend to apply for loans only (federal Stafford, Graduate PLUS, and/or private educational loans). I understand
that in order to apply for federal loans I am required to complete and file a Free Application for Federal Student
Aid (FAFSA) for 2021-2022, as well as a loan application(s) from a lender of my choice. To ensure timely loan
processing, you must send your completed loan application materials to the Law School Financial Aid Office,
and file your FAFSA, by no later than May 15, 2021. You are strongly encouraged to complete your FAFSA
online (https://studentaid.ed.gov/sa/fafsa), making certain to follow carefully the instructions regarding signature
requirements and to designate Columbia University as a recipient of your FAFSA information. If you are unable
to apply online and need a paper form, please contact the Federal Student Aid Information Center at 800-433-
3243.

I intend to apply for a need-based Law School grant, as well as for loans (see above for loan instructions). I
understand that in order to do so I am required to complete a CSS Profile application, as well as a FAFSA, and
these forms must be submitted for processing by February 15, 2021. In order to be considered for a grant, I must
also submit a copy of signed 2019 federal income tax forms, with all schedules, attachments, and W-2's, to the
College Board using the Institutional Documentation (IDOC) Service for all persons required on the CSS Profile
application [student, student’s spouse (if applicable), and both parents of student]. Students will be notified by
the College Board to submit tax returns using the Institutional Documentation (IDOC) Service shortly after the
CSS Profile form has been received by Columbia. Tax documents ARE NOT accepted by mail, fax or email. If
2019 taxes are not available by February 15, 2021, you must submit complete 2018 returns (via IDOC) by the
February 15, 2021 deadline, and 2019 returns by the end of April 2021 (again via IDOC).

directly for all questions about completing or submitting a CSS Profile application: 844-202-0524. The CSS
Profile application must be submitted to the processor by our February 15, 2021 deadline.
