The Railroad Retirement Board needs up-to-date information about the student's home
schooling to determine continued entitlement to benefits as a full-time student.

Required - A copy of the written notice to the school
district of intent to home school for the school year
indicated on Form G-315 including:
� the name, age, and grade level of the student;
� the address at which the instruction is done;
� the source of instruction materials;
� the basic core curriculum;
� proposed schedules; and,
� credentials of the home school teacher.
