CAMD Facility Work Request Form
About this form
This form is to be used to request services from or report problems to the CAMD Facility Manager.
The term "facility" refers to the CAMD Experimental Hall and adjoining Office buildings and most
assets permanently affixed to the building (e.g., walls, ceilings, doors, lights, heating and cooling, etc.).
For EMERGENCY work orders, call David Kleinpeter at (225) 229-3891.
For all other CAMD facility requests, please complete this form and email it
