Evidence for Vehicles to be Dismantled, Scrapped, or Destroyed (Form VTR-340).

Complete the “Metal Recycler Certification” following the vehicle’s permanent destruction, and surrender this form to
a TxDMV Regional Service Center before the 61
st
day following the permanent destruction of the motor vehicle on the
Surrendered Ownership Evidence for Vehicles Permanently Destroyed (Form VTR-340-M).
